With Surveys you build your own questionnaires with pages, media and branching, decide how anonymous responses are and how participants get in, and distribute them internally or by link. Responses form a history; thresholds raise alerts.

A form in four steps
1. BUILD — create pages and drop questions in. Besides scale, choice and text there are matrix, NPS, ranking, file upload and the info block for image, video or an embed. Any question can depend on an earlier answer. 2. CONFIGURE — anonymity, access, repeat responses, run time, consent text. 3. PUBLISH — from then on the content is frozen. 4. DISTRIBUTE — maintain recipients and generate links, or set a schedule rule.
How anonymous is anonymous?
ANONYMOUS means the response is stored with no person attached, the delivery link is dropped and the timestamp is rounded to the day — nobody can be traced, not even indirectly. The database enforces this, not the interface. PSEUDONYMOUS means a stable ID such as 'K-2087' instead of a name — still personal data under GDPR, but it carries a history over weeks. IDENTIFIED means by name, e.g. for an internal form.
How participants get in
SIGNED-IN USERS fill in inside the system. A PERSONAL LINK goes to one person, expires and can be revoked. An OPEN LINK works for everyone without an account — for NPS or an anonymous staff survey. ACCESS CODE lets people type an ID. Whether someone may answer more than once is a separate setting; saving and resuming later works on every route.
What you must observe legally
HEALTH DATA (therapy, care, strain) are special categories under Art. 9 GDPR: flag the form accordingly, obtain explicit consent and set a retention period. Withdrawal deletes that person's responses. STAFF SURVEYS are subject to works council co-determination (BetrVG § 87 (1) no. 6) and must be voluntary; the minimum group size blocks analysis and export for very small groups so nobody can be singled out. The default is five responses.
